All BUGS employees, students, families, and visitors seeking to enter the school building at 500 19th Street or participate in a BUGS facilitated Field Study trip must complete this health screening. This health screening must be completed on each day of arrival and results will reset at midnight of each day. Upon entering the facility or at the Field Study meeting location you will be asked to provide the results of your screening either by showing your phone or a printout of the results. Click the link: https://healthscreening.schools.nyc/ and follow the following prompts: 1. Click "Guest Screening" 2. Click "I'm a Student" and enter your child's information. The school or Facility You're Entering, select "Other" checkbox and enter "Brooklyn Urban Garden Charter School" in the text box, and press Enter. 3. Follow the Questions and Submit Screening when completed. 4. Your child must show via an email, screenshot, or printout "Cleared To Enter DOE Schools & Facilities" with date & time stamp to a BUGS Faculty member upon arrival to be allowed on Field Study trip. 5. Your child's temperature may be taken upon arrival. This form must be filled out the morning of your arrival. If your child exhibits any COVID symptoms, the temperature is higher than 100 degrees F, or is NOT "Cleared to Enter DOE Schools & Facilities" they may not go on the Field Study trip. We highly encourage students to take a COVID test prior to Field Study.
